A 83 year old man with 2-3 weeks history of worsening low back pain radiating to the right groin.
There is anklyosis of the sacro-iliac joints and most of the facet
joints. One set of facet joints are visible, between L2 and L3. There
is marked spondylotic change at this level with large osteophytes. The
lateral view shows squaring of the anterior surface of the vertebrae
and ossification of the anterior longitudinal ligament in the upper
visible vertebral bodies. The posterior surfaces are concave and appear
scalloped, especially mid-lumbar spine.
The appearance is really due to scalloping of the vertebral bodies and
is an indication of arachnoiditis with cyst formation, a complication
of this disorder.
